Surgical equipment repair request form
Important Notice: Equipment left in a faulty condition for extended periods may deteriorate further due to its delicate nature. The rate of deterioration will vary depending on the type and extent of the existing damage.
To help avoid additional delays and costs, we strongly recommend providing a repair decision within the 30-day quotation validity period. Equipment approved for repair after this period, or where further deterioration is identified during disassembly, may require reassessment and a revised quotation before repairs can proceed.
Assessment Fees & Equipment Purchase Option: If this quotation is declined and the equipment is returned without repair, an assessment fee may apply. SC Medical is also able to provide purchase offers for most medical equipment. If you would like to explore this option before having your equipment returned, please don't hesitate to contact us.
What happens after this form is submitted?
A Consignment Note will be emailed to you direct from TNT Express’ system.
Be sure your item for repair is packed and have it ready for the courier.
When we have received the goods, the item will be assessed and quoted. The quote will be forwarded to you via email. No repairs are carried out until your approval is received.
When repairs are approved, items will be repaired. Repairs will take anywhere from 24 hours to 3 weeks. An ETA can be given to you at the time of approval. In most cases, an Order Number is required with approval – if you do not use order numbers, just let us know.
When repairs are completed the goods will be shipped back to you.
